Who qualifies for enclomiphene?

Trial entry rules are not prescribing cutoffs. A proper evaluation confirms low testosterone, locates the likely cause and checks whether another diagnosis needs attention first.

Who is enclomiphene actually for?

Direct studies focused on men with secondary hypogonadism: compatible symptoms, repeatedly low testosterone and LH that was not elevated. Enclomiphene increases pituitary LH and FSH, so a response also requires functioning testes.1 That makes the lab pattern important, but no single testosterone, LH or FSH value determines eligibility by itself.

A dose-ranging study enrolled men with total testosterone below 350 ng/dL and LH below 12 IU/L.11 A later 16-week program enrolled overweight men aged 18 to 60 with two early-morning total testosterone results of 300 ng/dL or less and LH below 9.4 IU/L.2 These values describe study populations. They were not validated as universal clinical cutoffs, and the trials primarily measured hormones and sperm concentration rather than symptoms, pregnancy, live birth or long-term safety.

Who does enclomiphene not work for?

A clearly elevated LH with low testosterone points toward primary testicular dysfunction, where raising LH further is less likely to restore testosterone. FSH helps assess spermatogenesis and fertility, but LH is the guideline-recommended test for locating the androgen defect.5 A pooled 2025 analysis combined clomiphene and enclomiphene trials and found both gonadotropins increased versus placebo, but that pooled SERM result is not evidence of benefit in primary hypogonadism.3

A primary pattern is not a dead end; it is a different diagnosis with different treatments. Testicular injury, chemotherapy and some genetic conditions can cause this pattern. Testosterone replacement does not require the testes to increase their own output. The trade-offs of that path are covered in enclomiphene vs TRT. What a primary pattern should prompt is evaluation of the cause; evidence does not establish enclomiphene as effective treatment for primary testicular failure.

Direct enclomiphene studies did not establish benefit in men with normal testosterone. Both US guidelines diagnose testosterone deficiency only when compatible symptoms accompany consistently low results.45 An unexplained pattern may require further endocrine workup before any treatment decision. Enclomiphene is not FDA approved, and there is no authoritative US prescribing label that turns these findings into a complete eligibility or contraindication checklist.8

What testosterone level counts as low?

Guidelines use the result differently. The Endocrine Society cites a harmonized reference range of 264 to 916 ng/dL derived from healthy, nonobese men aged 19 to 39, while also directing clinicians to use accurate assays and laboratory reference ranges.4 The American Urological Association uses “a total testosterone level below 300 ng/dL as a reasonable cut-off in support of the diagnosis of low testosterone.”5 Neither number is a stand-alone diagnosis or an enclomiphene prescribing threshold.

On how to measure, the two agree completely. The Endocrine Society says clinicians should measure total testosterone “on two separate mornings when the patient is fasting.”4 The AUA says the diagnosis should be made “only after two total testosterone measurements are taken on separate occasions with both conducted in an early morning fashion.”5 Neither treats a single number as a diagnosis, and for good reason: by the Endocrine Society's own figure, about 30% of men with an initial testosterone in the low range are normal on repeat measurement.4

Variability is partly biological. When researchers drew repeated samples from the same men days apart, two measurements differed by more than 18 to 28% about half the time, and by more than 27 to 54% about a quarter of the time.6 The same study concluded that one sample is generally not enough to characterize a man's hormone levels. Testing twice reduces the chance that a transient result becomes a diagnosis.

Why must the blood draws be in the morning?

Testosterone commonly falls through the day, and an afternoon result can look low when morning measurements are normal. In a study of 66 men aged 30 to 80, estimated 4 p.m. values were 20 to 25% lower than 8 a.m. values at ages 30 to 40, with a smaller difference of about 10% at age 70.7 This is population-level variation, not a curve that can predict one person's result.

The same study measured how often an afternoon draw manufactures a false low: 17 men who dipped below 300 ng/dL on at least one afternoon measurement were normal at every one of their three morning visits.7 An afternoon number that would have started a treatment conversation simply disappeared once the draw moved to the morning. A provider happy to test you at 5 p.m. and prescribe from the result can create a misleading treatment conversation and should usually be confirmed under standard morning conditions.

Who should not take it regardless of labs?

There is no FDA-approved enclomiphene label defining a complete contraindication or monitoring list. FDA reviewers and the European Medicines Agency identified uncertainty around cardiac, thrombotic and ocular events in the development program, without establishing a comparative event rate or causality for compounded products.89 A prescriber therefore needs the full medical history, medication list, examination context and treatment goals, not a self-scored checklist.

Clomiphene labeling directs patients to stop treatment and obtain prompt ophthalmologic evaluation for visual symptoms. Applying that instruction to enclomiphene is a cautious class extrapolation, not a rule from an enclomiphene label.10 New visual symptoms or possible clot symptoms still warrant prompt medical assessment.

What does a proper eligibility workup look like?

Start with symptoms plus two properly timed total testosterone measurements. The Endocrine Society recommends fasting morning samples; the AUA requires two early-morning samples on separate occasions.45 Free testosterone can be useful when total testosterone is near the lower limit or sex hormone-binding globulin is altered.4

LH helps distinguish primary from secondary hypogonadism. FSH is particularly useful when spermatogenesis or fertility is part of the question. The AUA recommends prolactin when low testosterone is accompanied by low or low-normal LH.5 Pituitary MRI is not routine for every low result; AUA guidance supports it for total testosterone below 150 ng/dL with low or low-normal LH, regardless of prolactin, and further evaluation is warranted for persistent hyperprolactinemia or symptoms of a pituitary mass.

Potential contributors are worth reviewing because some are reversible without a prescription. Acute illness, sleep disruption, some medications and systemic conditions can lower testosterone. The clinician can use history and targeted testing to decide what applies. If the workup points at a reversible contributor, it should generally be addressed before or alongside drug decisions, with timing based on the clinical context.

And when the answer is no, the shape of the no tells you whether the provider was ever serious. A proper decline comes with an explanation and a direction: which finding needs attention and what follow-up is appropriate. A decline is not itself a diagnosis, and neither an approval nor a decline should outrun the workup. Is there an exact LH number that rules out enclomiphene?

No authoritative US enclomiphene label supplies an LH cutoff. Trials used LH below 12 IU/L or below 9.4 IU/L as enrollment criteria, not validated prescribing boundaries.

A high LH can indicate primary testicular dysfunction, while a borderline result needs clinical context and often further evaluation.

Can you take enclomiphene if your testosterone is normal?

Direct enclomiphene trials enrolled men with confirmed low testosterone, so benefit for men with normal testosterone has not been established.

Guidelines diagnose testosterone deficiency only when compatible symptoms accompany consistently low testosterone. Symptoms with normal results call for evaluation of other causes.

Low or low-normal LH with low testosterone can prompt prolactin testing, and severe secondary hypogonadism, persistent hyperprolactinemia or mass-effect symptoms can require pituitary evaluation.
