What is the difference between enclomiphene and TRT?
Testosterone replacement therapy supplies testosterone directly through a gel, injection, oral capsule or pellet. Enclomiphene blocks estrogen feedback at the hypothalamus and pituitary, raising LH and FSH so responsive testes receive a stronger signal to produce testosterone. One supplies the hormone; the other changes the signal that controls production. 2
TRT usually suppresses LH, FSH, intratesticular testosterone and sperm production, but the degree varies by person and regimen. In selected men with secondary hypogonadism, short enclomiphene trials found higher LH, FSH and testosterone while sperm concentration was maintained.3 That is a biochemical and semen finding, not proof of symptom relief, pregnancy or long-term safety.

Which raises testosterone more?
Short trials found similar average total testosterone with enclomiphene-family drugs and testosterone gel. That does not establish equivalence, and it says nothing direct about injections. A 2025 meta-analysis pooled clomiphene and enclomiphene: against placebo, total testosterone was 273.76 ng/dL higher on average, with a 95% confidence interval from 191.87 to 355.66 and high heterogeneity. Against gel, the mean difference was 5.41 ng/dL, with a confidence interval from 43.44 lower to 54.27 higher.1
In one 48-man dose-ranging study, day-42 morning testosterone averaged 604 plus or minus 160 ng/dL on enclomiphene 25 mg and 500 plus or minus 278 ng/dL on gel; the difference was not statistically significant.2 The pivotal trials did not establish improvement in libido, erectile function, energy, body composition, bone outcomes or quality of life.

Which is better if you want to stay fertile?
Exogenous testosterone can markedly suppress sperm production and should not be relied on as birth control. In a World Health Organization trial, 271 healthy fertile men received 200 mg of intramuscular testosterone enanthate weekly. In total, 157 men reached confirmed azoospermia; the cumulative estimate by six months was 65%. One pregnancy occurred during the azoospermic efficacy phase.5 That exact regimen and population are not the same as ordinary mixed-formulation TRT care.
In 16-week trials of overweight men aged 18 to 60 with secondary hypogonadism, enclomiphene maintained sperm concentration while testosterone gel reduced it.3 A pooled clomiphene and enclomiphene analysis estimated a 70.40 million/mL higher concentration than gel, with a 95% confidence interval from 41.62 to 99.18.1 These are semen results, not pregnancy or live-birth results. Guidelines recommend against testosterone therapy when near-term fertility is planned.7
What are the side effects of each?
The safety evidence is uneven. TRT has decades of use and large outcome trials. Direct enclomiphene evidence is shorter and too small to define rare or long-latency harms.
TRAVERSE enrolled 5,246 men aged 45 to 80 with symptoms, two fasting testosterone results below 300 ng/dL, and existing or high cardiovascular risk. With daily gel, major cardiac events occurred in 7.0% on testosterone and 7.3% on placebo over a mean 33 months of follow-up, meeting the study's non-inferiority criterion. Atrial fibrillation, acute kidney injury and pulmonary embolism were observed more often on testosterone.6 This was a gel trial, not an injection trial. FDA removed the class boxed cardiovascular warning in 2025 but required blood-pressure warnings for testosterone products.13
Short enclomiphene studies report headache, hot flush, nausea and muscle spasm. FDA and EMA reviews also noted cardiac, thromboembolic and ocular events, but the trials cannot establish causality or comparative rates.1014 Enclomiphene has no approved FDA label. The approved clomiphene label directs patients with visual symptoms to stop and obtain prompt ophthalmologic evaluation, and clinicians commonly apply that precaution to enclomiphene.4 Long-term clomiphene series are indirect evidence, not long-term enclomiphene safety.
Is enclomiphene FDA approved like TRT is?
No, and this is the difference most sales pages skip. Testosterone products are FDA-approved medicines with decades of labelling. Enclomiphene is not an approved drug in the United States: its manufacturer submitted it for approval as Androxal. In the sponsor's account of the 2015 FDA Complete Response Letter, the phase III design no longer adequately demonstrated clinical benefit and additional phase III work was requested. No enclomiphene product received FDA approval.9
What is sold today is compounded enclomiphene: made per prescription by compounding pharmacies, which means no FDA review of the capsule you actually receive. In 2022 an FDA advisory committee voted 4 yes and 8 no on recommending enclomiphene for the final 503A Bulks List. FDA's list updated 14 May 2026 still places it in Category 1, meaning under evaluation and potentially within conditional interim enforcement discretion.10 Category 1 is not FDA approval, endorsement or review of an individual compounded capsule. That does not make it illegitimate, but it is a real difference in oversight, and a provider who does not volunteer it is telling you something. Which costs less?
There is no universal cash-price winner. Current enclomiphene programs mix medication, memberships, laboratory testing, visits and multi-month commitments in different ways, so a monthly equivalent can conceal both upfront cash and separately billed care. Coverage varies by plan, pharmacy, dose and benefit rules. Generic testosterone injections may be covered while compounded enclomiphene is often paid in cash, but an individual benefits check is the only reliable comparison.

Who is each treatment actually for?
Enclomiphene can only amplify a signal that the testes can answer. The pivotal randomized program studied selected men with secondary hypogonadism, shown by low testosterone with low or inappropriately normal LH.2 In primary hypogonadism the testes themselves have failed, which shows as low testosterone with high LH and FSH. The pituitary is already shouting, and a drug that makes it shout louder cannot fix testes that cannot answer.
That is why a proper workup measures LH before anyone chooses a therapy, and why an honest enclomiphene provider will decline a share of the men who apply. Testosterone therapy bypasses that signalling wall by supplying exogenous testosterone rather than asking the testes to increase output. That mechanistic difference does not establish eligibility; diagnosis, contraindications and individual risks still govern treatment. What has direct evidence, and what does not?
The useful comparison is outcome by outcome. A blank is as important as a result.

Can you switch between them?
Yes, in both directions, and both switches belong to a clinician rather than a checkout page. If enclomiphene does not move your levels after a fair trial at an adequate dose, the first question is the diagnosis, not the dose: elevated LH suggests primary testicular dysfunction and a lower likelihood of SERM response, while FSH adds information about spermatogenesis. Re-checking the pattern matters more than pushing harder. For confirmed non-responders, a clinician may consider testosterone therapy, which supplies testosterone without relying on endogenous production.
No controlled trial establishes a TRT-to-enclomiphene switching protocol. In one 12-man direct study, total testosterone returned to pretreatment values one month after enclomiphene stopped; another trial found hormonal effects persisted for at least one week.212 Those small studies do not establish a post-TRT recovery schedule. How do you choose a provider for either?
By how they test, not what they sell. When researchers secret-shopped seven direct-to-consumer testosterone platforms with a fictional 34-year-old whose total testosterone was 675 ng/dL and who wanted future fertility, six of seven offered him testosterone and none used two separate early-morning testosterone measurements.811 Five of the six prescribing platforms never discussed the red-blood-cell risk that TRT monitoring exists for.8
A defensible workup starts with symptoms plus two separate early-morning testosterone measurements, fasting under Endocrine Society guidance.7 Free testosterone is added when total testosterone is borderline or SHBG may be altered. LH helps locate the defect; prolactin follows low testosterone with low or low-normal LH; FSH and semen analysis matter when fertility does. The plan should also state monitoring, regulatory status and total price.
Common questions
Can you take enclomiphene and TRT at the same time?
Enclomiphene plus TRT is not an evidence-based routine combination. TRT suppresses the LH and FSH signal enclomiphene is intended to increase, and no controlled trial establishes clinical or fertility benefit from combining them.
A clinician should own any combination decision.
Does testosterone crash after you stop enclomiphene?
Small direct studies give only a short window. One trial found the hormonal effect persisted for at least a week after the last dose; a separate 12-man study found total testosterone had returned to pretreatment values one month after treatment.
Neither establishes a general discontinuation schedule.
Why do some men feel no different on enclomiphene even when testosterone rises?
A biochemical response does not prove symptom benefit. Direct enclomiphene trials established hormone and semen endpoints, not reliable improvements in energy, libido or erectile function.
If symptoms persist, a clinician can reassess the diagnosis, free testosterone when indicated, adverse effects and other causes rather than assuming a higher dose will help.
Is enclomiphene the same thing as Clomid?
No. Clomid contains both enclomiphene and zuclomiphene, while isolated enclomiphene contains one isomer. Zuclomiphene persists longer and has different estrogenic activity, but clinical outcome differences between isolated enclomiphene and Clomid remain incompletely established.
