Enclomiphene vs hCG

Enclomiphene turns up pituitary signals; hCG activates the LH receptor at the testes. The useful comparison changes with the diagnosis and whether the endpoint is hormones, sperm production or pregnancy.

What is the difference between enclomiphene and hCG?

Enclomiphene works upstream. It is a daily tablet that blocks estrogen feedback at the hypothalamus and pituitary, so the brain raises its output of both signalling hormones, LH and FSH, the two lines of instruction the testes run on.1 hCG works downstream: it is an injected hormone similar enough to LH to activate the same receptor on the testes directly, driving testosterone production without the brain's involvement. One turns up both of the brain's signals; the other replaces one of them from outside.

What they share is what separates both from testosterone therapy: the testes remain the source of testosterone rather than receiving testosterone from outside, the contrast covered in our enclomiphene vs TRT comparison. But the two are not interchangeable. hCG does not replace FSH, and endogenous gonadotropins can remain low in men with hypogonadotropic hypogonadism. A 13-man study in that severe population measured LH and FSH below the assay limit during hCG-only maintenance.2 That result should not be generalized to every man who uses hCG.

One boundary applies to both: neither is expected to correct primary testicular failure, where the testes cannot adequately answer the signal. Enclomiphene needs one thing more, a pituitary able to raise its output, which is why it has been studied and used in selected men with secondary hypogonadism and why an honest provider declines a share of the men who apply. hCG has a complementary mechanism: because it bypasses pituitary LH, it has long been used in selected gonadotropin-deficient men with retained Leydig-cell function.2

Which raises testosterone more?

No direct answer exists, and it is worth saying plainly that no randomized trial has ever compared enclomiphene head-to-head with hCG. The closest evidence is a 282-man randomized trial of the sister drug clomiphene: men with low testosterone who wanted to preserve fertility received clomiphene 50 mg daily, hCG 5,000 IU twice weekly, or both together. At three months, testosterone had risen in all three arms, with no statistically significant final-testosterone difference among groups. Pooled mean testosterone rose from 2.31 to 5.17 nmol/L, an increase of 2.86 nmol/L or about 124% from baseline.3

Two caveats belong next to that result. Clomiphene is not enclomiphene: it is the mixture from which enclomiphene is purified, close kin but not the same product, and the practical differences are covered in enclomiphene vs Clomid. The pooled mean on-treatment value was 5.17 nmol/L, roughly 149 ng/dL, but that absolute value cannot be assigned to an individual arm.3 A 2025 review reproduced arm-level changes from that same study; it did not create a pooled EC-versus-hCG comparison.4 The evidence supports no enclomiphene-versus-hCG ranking.

Which has direct fertility evidence?

The answer depends on the population. Enclomiphene raises LH and FSH in men with secondary hypogonadism.1 But hormone changes and semen measures are surrogate endpoints. Enclomiphene has not demonstrated pregnancy or live-birth benefit.

hCG-based gonadotropin treatment has more direct fertility evidence in a different and more severe group. A 2026 meta-analysis included 50 studies and 1,583 azoospermic adult men with hypogonadotropic hypogonadism. Spermatogenesis was achieved in 74% (95% CI 66% to 82%) after a mean 11.8 months. Among couples with a child wish, pregnancy occurred in 52% (95% CI 42% to 62%) after a mean 19 months. Combined hCG plus FSH achieved spermatogenesis in 80% versus 53% with hCG alone.8 The studies were heterogeneous and most were of poor methodological quality, but their endpoints are more direct than hormone normalization.

A smaller mechanistic study followed 13 men with severe hypogonadotropic hypogonadism after sperm production had been established with combination hormones. During hCG-only maintenance, sperm remained present in all but one man but counts declined, supporting an important role for FSH in quantitatively normal sperm production in that population.2

A separate question is whether hCG preserves fertility alongside TRT. Outside testosterone suppresses the brain's signals and with them sperm production, the problem covered in enclomiphene vs testosterone injections. A retrospective series of 26 men on TRT with hCG 500 IU every other day reported maintained semen parameters and no azoospermia.5 The AUA and ASRM guideline nevertheless concludes that evidence for adding hCG or other agents to exogenous testosterone to preserve spermatogenesis is too limited to recommend.9

What does taking each actually involve?

Enclomiphene is a tablet. Current US urinary hCG product labels specify intramuscular injection; subcutaneous administration appears in specialist studies and protocols and is off-label. The randomized study above used 5,000 IU twice weekly,3 while the retrospective TRT-adjunct series used 500 IU every other day.5 Those are study-specific regimens, not a single typical dose.

The logistics differ more than the schedules suggest. hCG arrives as a powder that has to be reconstituted with diluent, refrigerated once mixed, drawn up and injected on schedule, with syringes and swabs as standing supplies. None of that is difficult, and men on fertility protocols manage it routinely, but shelf life after mixing is product-specific: current Novarel labeling says 30 days and Pregnyl says 60 days.1011 A tablet is a tablet. The delivery route says nothing about which drug fits your biology; it is simply a real difference in what your week looks like.

hCG is regulated as a biologic. On March 23, 2020, the FDA's rule defining the term biological product took effect, and approved hCG products, previously regulated as ordinary drugs, were deemed licensed biologics from that date.6 The compounding exemptions in US drug law do not reach biologics. That means the ordinary 503A and 503B bulk-drug exemptions do not authorize making hCG from a bulk substance outside a biologics licence.7 FDA separately describes limited enforcement discretion for certain mixing, diluting or repackaging of an already licensed biologic. That is not bulk compounding.

The precise question for any offered product is whether it is an approved hCG product, a permitted handling of an already licensed biologic, or a product made from bulk substance without a BLA. Calling all three “compounded hCG” hides the distinction.

Enclomiphene is not FDA approved. FDA and EMA reviewed its development program, and regulators found that hormone and semen surrogates did not establish meaningful clinical benefit. EMA also cited venous thromboembolism cases, including one fatal event, while causality and incidence remained uncertain.1213 It remains in FDA Category 1 for patient-specific compounding under interim enforcement discretion, which is not approval.14

Which costs more?

A vial price is not a monthly cost until a regimen is specified. How far a vial stretches depends on dose: the 500 IU every-other-day schedule from the retrospective TRT-adjunct series makes a vial last over a month,5 while the 5,000 IU twice-weekly dose from the randomized trial uses one up in about a week.3

Prices for both options vary by product, quantity, pharmacy, location, coupon, insurance and whether labs or clinical services are bundled. Compare the same date, dose and included services. Can they be used together?

Clinicians sometimes combine an upstream SERM with hCG, but the randomized evidence is for clomiphene plus hCG, not enclomiphene plus hCG. In the 282-man trial, the combination raised testosterone no more than either drug alone, but it scored best where it counts to the man taking it: symptom improvement on the qADAM scale at three months was 15.13 points with the combination, against 12.73 on clomiphene alone and 11.82 on hCG alone, a statistically significant advantage.3

The combination has a plausible mechanism, but keep the result in proportion. It is one trial of clomiphene, not enclomiphene, with a symptom questionnaire as the winning margin. Combining or sequencing these drugs is specialist territory, not a checkout option, and that includes the popular idea of using them to restart natural production after TRT, where no controlled protocol has been established. A clinician who has seen your LH, FSH and semen analysis decides; a pricing page does not.

Which option fits which clinical goal?

The evidence changes with the diagnosis, so a goal-by-phenotype map is more useful than one winner.

Common questions

Does hCG raise FSH?

No. hCG activates the LH receptor and does not replace FSH.

In a 13-man study of severe hypogonadotropic hypogonadism, sperm counts declined during hCG-only maintenance after spermatogenesis had first been induced with combination hormones. That finding supports a role for FSH in quantitatively normal sperm production in that specific population, not a universal result for every hCG user.

Is compounded hCG legal?

The ordinary 503A and 503B bulk-drug compounding exemptions do not authorize making hCG from bulk substance because hCG is regulated as a biologic.

FDA separately describes limited enforcement discretion for certain mixing, diluting or repackaging of an already licensed biologic. That is not the same as a bulk-compounded hCG product.

Which works faster, enclomiphene or hCG?

No head-to-head trial has timed enclomiphene against hCG.

Separate studies used different drugs, populations, doses and visit schedules, so their time points cannot establish which works faster. Hormone movement also does not establish the timing or likelihood of symptom improvement.

Why do TRT clinics prescribe hCG alongside testosterone?

TRT suppresses endogenous gonadotropins, so clinicians sometimes add hCG to stimulate the LH receptor.

A 26-man retrospective series reported maintained semen parameters with TRT plus low-dose hCG, but the AUA and ASRM guideline says evidence for adding hCG or other agents to exogenous testosterone to preserve spermatogenesis is too limited to recommend.
